loud hum / buzz / ground issue?

Hello Everyone,
Have put in a bunch of time on the build and have a loud buzz coming off the main outs.
As well as the BD, which is the only voice i have built. Planning to build the Noise in the meantime.
I'm getting 5 volts off of the 1 leg of the Test Mode on of the processor, and no sound from the BD.
I am also unable to get any 13v at TP1. However I'm getting 15v on the VCC+ input of the (IC6) TL072P DAC.
Anyone have any recommendations on how to address the hum / buzz?
I'm getting 15v & 5v on the IO Board and from there everything looked good.
