Building IO Board - Measuring wrong voltages

I've stupidly started building the Yocto 2 without following the assembly guide and only now got to testing what I've built.
I've finished the whole IO board, but I'm measuring 18.35v where I should be getting 15v, and I'm only measuring 1.5v between J10 pin1 GND and pin8 5v.

I've checked and double checked my soldering and replaced both REG1 and REG2 but I keep measuring those same voltages.
Can anyone please point me in the right direction in order to start troubleshooting?

You are sure your power supply (adapter) is an AC one?
Min 16V and 1A AC
